About Us

Scott Dennis founded From the Heart Home Care in 2001. Prior to that Scott served in the Sheriff's Department in Placer County for 3 1/2 years.

Since 1991 he has been devoted to senior support services. Scott is a frequent guest speaker on the topic of the challenges of senior care. He has been active with The Council on Aging, Continuity of Care, the Elder Abuse Protection Council, Big Brothers and Sisters, Section on Aging, the Alzheimer's Task Force, Local Hospitals, and the Chamber of Commerce. From the Heart Home Care is also members of National Hospice and the Palliative Care Organization.

Office Team

Lisa Harrison is our team leader for all client and caregiver interactions. Working closely with Angela Froschl, Office Manager in Santa Rosa, Vanessa Martinez in San Rafael & San Francisco, Nicole Merserau in Dublin, and Eleanor Vasquez for San Mateo and Sunnyvale.  Lisa has worked with Scott Dennis since 1997 and has a degree in kinesiology (the study of human movement) equipping her to make the right match for every client.

Placement Services
Stacy Woltz - Placement Director.  Stacy experienced her grandmothers care needs and the difficult decision process.  She feels blessed to have opportunity to assist families with some of the difficult decisions she faced.  When a family comes to her for help, whether it is in preparation for the future or in the midst of crisis, she loves the opportunity to offer them her expertise and help them discover a courage they didn't know they had.

Lori Schmieding - Placement Director.  Lori has experienced many of the same is challenges, stresses and fears families might be facing.  She is thrilled to be able to use her knowledge and experience to help you in making the very best decision regarding the care of your loved one. Her hope is that you too will experience the relief that come from knowing you have found the best care possible for your loved one.

Deanna Shaat - Placement Director.  Deanna has worked with and in assisted living communities for several years.  She has seen the wide range of care needs and is very sympathetic to the individuals.  Using her understanding and compassion has made her a trusted advisor.

Sonoma and North Bay Counties
Alexis Baier - Market Director in Sonoma County.  Alexis is very focused on client needs and is very responsive.  She brings energy, compasion, and a love to help.

Marin County
Tony Williams - Our partner and director in Marin County, Tony brings a strong background of customer service. Having spent his career in the insurance industry, Tony is skilled in listening to and understanding the specific needs of each client. Working closely with Vanessa Martinez, our Marin client liaison, they strive to make the right match of skills, personality, and professionalism.

San Francisco County

Alameda/Contra Costa Counties
Leizl Pagtaconan - Leizl is the Marketing Director for the East Bay region, covering from Pleasant Hill to Livermore.  She is an extremely hard-working person, who prides herself on her attention to detail and her ability to work well with others.In her spare time she enjoys volunteering in the community. She has volunteered at numerous organizations including the American Heart Association, the Fred Biletnikoff Foundation, and Make-A-Wish Foundation.

Santa Clara County
Matt Mukoyama, director and partner of the South Bay office, brings enthusiasm and professionalism to the area. Having personally experienced the difficulties that many of our client families go through, bringing understanding to each client.

San Mateo County
Ray Liu, director and partner for the Peninsula office, has personal experience in caring for loved ones.  Ray's experience, professionalism and empathy bring understanding and care to our families.
